| | If you would like, the patient information form can be downloaded here. You may print this form, fill it out, and bring it to your first appointment to save time. |
| | We ask that you have as much of your insurance information as possible (ie. your plan booklet, insurance card, etc.). We will gladly bill your insurance company for your visit, so it is helpful to have as much information as possible. A phone number for your insurance company will also help us so we can call and get a general descriptoin of your plan benefits. |
| | If you require pre-medication prior to a dental treatment, we ask that you have your physician prescribe the pre-medication for you until we have seen you in our office as an established patient. |
| | If any of your information has changed, please inform us prior to your appointment. These changes include, but not limited to, change of address, changes to your insurance, and changes in your general health (including allergies). |
| | If you require pre-medication prior to dental treatment, we ask that you please let us know in a sufficient amount of time, so that we may phone in the prescription. |
| | We ask that you have your portion of the payment at the time of the visit. We accept Visa, Mastercard, Discover, and have financing options available through Wells Fargo Bank. |
| | We ask that anyone under 18 years of age is accompanied by a parent or guardian. |
| | If you are having any specialty work done in our office (ie. root canal, extractions, etc.), we ask that you eat before coming in for your appointment. |
| | If you have any current x-rays taken by another office, we ask that you bring those in for your appointment. |
| | If you have any medical conditions that we may need to speak with your physician about prior to treatment, please bring the phone number. |
